EPP 1: Mandating Health Insurance for Foreign Workers

Approximately 3.1 million foreign workers employed in low-technology, labour-intensive industries in Malaysia are uninsured or under-insured, resulting in a substantial amount of unpaid hospitalisation bills at public hospitals. Through this EPP, private medical insurance-- the Hospitalisation and Surgical Scheme for Foreign Workers (SPIKPA)--was made mandatory for all foreign workers, with the exception of domestic maids and plantation workers, to reduce the strain on Malaysia’s public healthcare system.


  • In 2014, 1.7 million foreign workers were insured under the Foreign Worker's Health Insurance Protection Scheme (SPIKPA).
  • The Cabinet has agreed that the registration of Third Party Claims Administrators will be placed under the purview of Bank Negara Malaysia to ensure the smooth implementation of the scheme.
